This form is used for requesting official school documents for Preschool, Elementary, Junior High School, and SPED. 

CONFERENCE APPOINTMENT REQUEST FORM

For Preschool, Elementary, Junior High School, SPED, Senior High School

This form is used for requesting a conference appointment (online, on-site, or via phone call) with a teacher, counselor, or administrator. 

STUDENT/PARENT INFORMATION UPDATING

For Preschool, Elementary, Junior High School, SPED, Senior High School

This form is used for requests for student information corrections or updating such as misspelled names, names of guradians/parents, contact number updating, etc.

STUDENT GENERAL REPORT FORM

For Preschool, Elementary, Junior High School, SPED, Senior High School

This form is used to report cases of bullying/cyberbullying, cheating, vandalism, littering, harassment, teacher not teaching/checking correctly, Covid-9, etc. This is also where comments and suggestions may be sent.

APPEAL FOR MAKE-UP BLOCK SUMMATIVE TEST 

This form is used for appeals to be allowed to take a make-up summative test. Please take note that the Summative Test is different from the Major Exam. This form requires the submission of a documentary proof such as medical certificate, etc. 

APPEAL FOR MAKE-UP EXAM

This form is used for appeals to be allowed to take a make-up major exam exam. Please take note that the Major Exam Summative Test is different from the Summative Test. This form requires the submission of a documentary proof such as medical certificate, etc. 

APPEAL FOR MAKE-UP LEARNING ACTIVITY 

This form is used for appeals to be allowed to take a make-up LA. Please take note that the late LA submission appeal is different from the makeup LA. This form requires the submission of a documentary proof such as medical certificate, etc. 

GFA Birthday Celebration Form

This form is designed to help the GFA Department coordinate and schedule your child’s birthday party celebration on campus. Please use this form to request a date and provide necessary details about the celebration.

TRANSFER / DROPPING OUT FORM

For Preschool, Elementary, Junior High, SPED, Senior High School

This form is for students who have decided to officially transfer or drop out.

Request to Conduct a Research

This form is used to request to conduct a research about GFA. You will receive an email regarding your request within 5–7 business days. 

Tutorial Services Registration

This form is used to request to avail tutorial services conducted by GFA Faculty & Staff. 

APPEAL FOR EXCUSED ABSENCE

For Preschool

This form is for parents or guardians of preschool students who are requesting that an absence be excused due to special circumstances. 

HEALTH AND GUIDANCE FORM

This form is used to collect important health information about the students.
